Creating ideal soft tissue around implants
Soft tissue is one of the most important aspects of implant therapy. It not only creates a functional barrier against the bacterial challenges and protects the bone from resorption but it is also a critical part of successful aesthetic treatment. Treatment planning should also include soft tissue manipulation from the very first incision so that ideal soft tissue can be achieved in a predictable manner. In dealing with the issues of soft tissue at the start you will reduce the need for corrective surgery at a later stage and improve your success rate and your patients acceptance. This lecture will go through a number of techniques in order to maximise the soft tissue results for both functional and aesthetic cases. We will give hints and tips on all aspects of soft tissue management which you can take back to your practice.
Correcting soft tissue defects around implants
Implants have become a standard of care in dentistry. However often the rules are not closely followed and our patients end up with treatment outcomes that are well below their expectations. Dealing with these complications is both a challenging and emotional issue for the surgeon and patient. It is critical to have the correct tools at our disposal in order to correctly manage these problems so as to get the best result for our patient. This lecture will deal with the causative issues related to failed aesthetics. We will also cover all aspects of soft tissue grafting needed to correct soft tissue defects around implants and provide hints and tips with regards to management of soft tissue and suturing for these cases.
Harvesting Tissue from the palate
Taking tissue from the palate can be the most difficult part of soft tissue grafting. Removing the right length and thickness of tissue is critical to good coverage and failure to do this can lead to total failure of your technique or only a partially successful result. Do deep palates provide the best harvest site and are there different sites and techniques that can be used for harvesting soft tissue from the palate? How do we harvest connective tissue versus a free gingival graft? How do I deal with the complications of harvesting tissue from the palate? All these questions and more are answered in this lecture as well as a lot of tips and tricks to make the treatment as painless for the patient as possible.
Hands-on Outline
Pouch Technique – The Pouch Technique is a soft tissue CTE technique that allows us to increase the thickness of the tissue. This technique can be used for immediate implant placement or even as a soft tissue graft for a pontic site.
Apical Repositioned Flap – The apically repositioned flap is one of the most important flaps available to both increase the tissue thickness and height as well as expand the band of attached gingiva. This hands-on will show you how to successfully split flaps and reposition this using a microsurgical technique to improve your soft tissue around implants.
The Vestibuloplasty with a free gingival graft is a critical technique to master if you are going to place implants. Attached gingiva is critical to the long term health of the implants and this is one of the few techniques possible to effectively do this.
Dr Gluckman completed his dental training at the University of Witwatersrand in Johannesburg in 1990. After spending a number of years in a general practice he completed a 4-year full time degree in Oral Medicine and Periodontics at the University of Stellenbosch in Cape Town, which he completed with distinction (cum Laude) in 1998.
He was intimately involved in the development of the postgraduate diploma in Implantology at both the University of Stellenbosch and later at the University of Western Cape. He is currently in full time private practice in Cape Town.
Dr Gluckman was the author of a monthly Implantology Corner for the South African Dental Journal and is the immediate past president of the South African Society for Dental Implantology and on the board of the Southern African Association of Osseointegration (SAAO). He has served as the secretary of the South African Society of Periodontics.
He is on the experts panel of the international educational website Dental XP and is on the Dental XP scientific board. He is the director of the Implant and Aesthetic Academy (IAA), which is a private post graduate training facility in South Africa which provides education in implantology and aesthetic dentistry. The Academy is accredited by The University of Frankfurt. He has been involved in implantology training for 18 years.
His special interests are immediate placement/immediate load as well as soft tissue aesthetics and periodontal plastic surgery, autogenous bone augmentation especially bone harvested from the palate and three-dimensional bone augmentation.
